Signature Update Instructions

Having an updated signature on file will allow us to process Absentee Ballots more accurately.  We are utilizing the standard signed Voter Registration Application form to obtain a current signature.  Using the standard Voter Registration Application form also allow us to update any other information that may have changed since the original form was processed.  Please fill out the form in its entirety even though some of the original information may not have changed. 

After filling out the online form, please print it on your printer and mail it back to the Supervisor of Elections office at:

If you do not have a printer, please email us at elections@slcelections.com or call 772-462-1500 and a Voter Registration Form will be mailed to you.
